We can’t wait to welcome you. To help your first visit go smoothly, here are a few things to keep in mind:
- Â Seeing a chiropractor, physiotherapist, athletic therapist or kinesiologist? Please bring shorts so we can assess movement comfortably.
- Insurance billing: If you’d like us to bill your insurance directly, please bring your benefits card. We also require a credit card on file for all patients.
- Online forms: If you haven’t had a chance to fill out our online forms (available here), please make sure you arrive at least 15min prior to your scheduled appointment time to complete them in person.
- Cancellations: Your appointment time is reserved just for you. Please give at least 24 hours’ notice to avoid a 100% cancellation fee.
Note: Third-party payers do not cover cancellation fees.

You and your practitioner have equal roles in your recovery, rehabilitation, and return to normal function.
Your responsibilities to your practitioner are to provide…
- a cooperative commitment to your treatment plan
- compliance with office policies, procedures and fees
- honest, accurate and full disclosure of all pertinent health information
- constructive feedback (positive/negative) regarding all aspects of care
- courtesy and respect for the office environment, staff and other patients
- up-to-date contact information
 You have the right to expect your practitioner to provide…
- ethical conduct of practice
- respectful, honest and clear communication in all aspects of consultation, consent, examination and care
- full disclosure of policies, procedures and fees
- relevant, safe and supportive patient-centred care
- accurate and comprehensive records
- a private, clean, accessible setting for receiving care, that respects patient wishes
- accommodation and accessibility for disabilities and human rights, or alternative arrangements if accessibility is not possible
- an awareness of current health and well-being issues
- timely and necessary communication and/or referral to other health professionals
- compliance with their respective College or Association regulations, standards of practice, policies and guidelines
- privacy and confidentiality of your personal health information
- behaviour and clarity regarding dignified professional boundaries
- disclosure of real or perceived conflicts of interest
